Accidents cost lives and waste money. It is in everyone's interest that the industry understands why accidents happen: not just the immediate cause but the several underlying layers of cause. Insurers are at the forefront of this. Time and again they identify the key to reducing the number of casualties the establishment of a sustainable safety culture both on shore and on board. So why do insurers such as The Swedish Club have to restate the need for an entire organisation to be committed to casualty prevention?
