Accidents occur

The reasons humans have developed to explain why accidents occur has changed over time, said Ossur Hilduberg, head of Danish Maritime Accident Investigation Board (DMAIB), on the SHW podcast at SEA. Going back about 100 years, many people would say accidents were simply God's will. The simple explanation as to why seafarers were injured or ships were grounded was the work of divine forces - a great get out clause to avoid finding solutions to prevent similar incidents from occurring in the future. From the 1920s onwards, a more research-based approach to understanding the root causes of accidents became the norm, moving away from religious or supernatural explanations. This is around the time that the term 'human error' began to be used more commonly when talking about accidents.
