It's something of a truism to say that safety-critical industries have to deal with occupational hazards. Transport, process control, oil and gas, nuclear and petrochemical industries, to name but a few, all have procedures and measures in place to mitigate risks to their workforce, their users, and the public. At the same time, they are under constant pressure - from their stakeholders as much as their consumers -to maximise productivity. However, these two sub-goals - production and protection -are commonly seen to be in opposition. An increase in one aspect apparently leads to an inexorable decrease in the other.1.
