In its report on the derailment of a coal train at Crewe Junction at the north end of Shrewsbury station on July 7 2012, RAIB exposes what amounts to a local breakdown in Network Rail's track maintenance and inspection regimes. Had these been properly implemented, they would almost certainly have identified and corrected the hazard that No 75 points represented. The train operated by Freight-liner was the 07.25 from Portbury Dock to Fiddlers Ferry, and the wagon which derailed was a type HXA. The only fault found with the wagon was a slight imbalance in the wheel loads across the wheelset, but this was a minor factor.
