According to OSHA, the Control of Hazardous Energy, Lockout/Tagout was the second most cited standard for manufacturers from October 2001 to September 2002. Violations for lock-out/tagout (LOTO) remain high, despite the fact that the rules were promulgated more than a decade ago. Implementing LOTO is not difficult, but many companies wrestle with consistency―employees simply don't follow procedures. Often, employees feel comfortable working around large machinery and become complacent about following LOTO. They frequently adhere to the notion, "It can't happen to me," or "This will just take a minute."
