Plymouth Hospitals NHS Trust has been ordered to pay £19,228 in fines and costs after it failed to take appropriate measures to reduce contamination when a radioactive implant was damaged in an operating theatre. Magistrates heard the damaged implant or 'seed', which is used in the treatment of prostate cancer, should have been spotted straight away at Derriford Hospital in Plymouth in October 2008, but it wasn't j until 43 days later when radioactive contamination was picked up during a routine annual inspection that the Trust took action to clean up.
