The siemaszko case has been settled by the Nuclear Regulatory Commission staff, and the settlement was approved in July by an Atomic Safety and Licensing Board. Andrew Siemaszko was the engineer responsible for vessel head cleaning during a refueling outage in 2000 at FirstEnergy Nuclear Operating Company's Davis-Besse reactor near Oak Harbor, Ohio. In the investigation of the erosion of the reactor vessel head, which was made known to the NRC in 2002, the NRC found that Siemaszko had provided incomplete and inaccurate information regarding the cleaning and the condition of the vessel head. The NRC's original order in the case, issued in April 2005, barred Siemaszko from employment in NRC-regulated activities for five years.
