A US contractor who repeatedly breached safety standards faces steep fines and a violator listing. US contractor, P. Gioioso & Sons Inc., has been cited for four wilful violations involving a lack of cave-in protection and a safe means of escape at the contractor's work sites in Cambridge and Framingham, Massachusetts.
