NÉOPLASIE ENDOCRINIENNE MULTIPLE DE TYPE 1. BILANS DE DÉPISTAGE ET DE SURVEILLANCE DES ENDOCRINOPATHIES ASSOCIÉES AUX TUMEURS DU PANCRÉAS

A patient with an endocrine pancreas tumor is at great risk for having MEN 1 (30 to 60% ofgastrinomas, 4 to 10% of insulinomas). Prospective screening for MEN 1 comprises measurement of blood calcium and PTH 1-84 levels (hyperparathyroidism is present in 90% of MEN 1 patients), pituitary imaging and few biochemical measurements (a pituitary adenoma is encountered in 20 to 60% of MEN 1 patients) and lastly adrenal imaging (an adenoma is found in one-third of MEN 1 patints). When the diagnosis of MEN 1 is made in a given patient (clinically and/or genetically) the screening procedure aiming at identification of other endocrine abnormalities must be undertaken yearly for biochemical tests and every 3 or 5 years for radiological procedures.%La néoplasie endocrine multiple de type 1 (NEM 1) est fréquente en cas de tumeur endocrine pancréatique (30 à 60% des gastrinomes, 4 à 10% des insulinomes...). La recherche d'une NEM comportera une calcémie ionisée et un dosage de la PTH 1-84 (l'hyperparathy-roïdie est présente dans 90% des NEM 1), une imagerie hypophysaire et quelques dosages hormonaux simples (un adénome hypophysaire est retrouvé dans 20 à 60% des NEM 1) ainsi qu'un scanner des surrénales (une tumeur est présente dans 1/3 des NEM 1). Lorsque la NEM 1 est prouvée (cliniquement ou génétiquement), le bilan biologique de dépistage des diverses atteintes endocriniennes est effectué une fois par an, les examens radiologiques une fois tous les 3 à 5 ans.
