In January this year while the 114 809dwt Isle of Man-registered tanker British Mallard was docked in Kwinana, Western Australia, the ship's engineers were attempting to rectify a crew lift fault. According to the casualty investigation report completed by Australia's Transport Safety Bureau (ATSB), after making some adjustments to the second deck landing door switches the electrical technician stepped onto the ladder in the lift shaft. He then asked the second engineer to close the doors behind him.
