Ⅱ. Blackout incident due to air ingress in the central cooling fresh water system(CCFW)

On 16th Feb 2012, the captioned vessel observed water overflowing from the CCFW expansion tank in bursts. The vessel was slowed down and stopped, but due to a large amount of air in the water system, two diesel generators tripped. The standby diesel generators started, but they also tripped on high jacket water temperature. Subsequently, the emergency generator started and the vessel was on emergency power. The overflowing ceased after some time and the level of the expansion tank dropped below the sight glass. No abnormality was observed on the diesel generators and main engine.
