PENALTY VIOLATIONS

This issue's reporting period for penalties issued by the US D.O.T., Pipeline and Hazardous Materials Safety Administration covers September and October 2019. Penalties in the form of Compromise Orders, Default Orders, Notices of Probable Violation, and Final Orders were issued to 46 companies. Violations of the HMR covered many areas of the hazardous materials regulations. Highlighted recurring issues are those violations committed by failing to provide both initial and recurrent hazardous materials employee training, as well as failing to close packages in accordance with instructions provided by the packaging manufacturer.
