Patients' Identification and Reporting of Unsafe Events at Six Hospitals in Japan

Hospitals and other health care organizations have increasingly recognized the need to engage patients as participants in patient safety initiatives,~(1,2) as reflected in a burgeoning literature on patient participation in general, and in patient reporting of safety events in particular.3 In a previous study, we found that 10.6% of 1,207 inpatients admitted to three acute care hospitals in Japan had experienced "unsafe" or "uneasy-dissatisfying" events and that health care staff identified or reported only 24.5% of unsafe events.4 Similar findings regarding patients' and health care staff's identification of adverse events can be found elsewhere.~(5,6) We conducted the current study to further compare patients' and health care staff's identification and reporting of adverse events.
