Mrs.Smith is a 72-year-old woman who was admitted to the hospital with community-acquired pneumonia (CAP).On Day 2 of her hospital stay,the physicians discussed that she might benefit from a pneumococcal vaccination (PCV),given her age and diagnosis,but they needed to first review her history.The order was not written during the resident rounds,and the primary resident rotated off service without writing the order for the vaccine.Mrs.Smith was discharged home the following week without being screened for the appropriate vaccination.
