At Vidant Health (VH) a private, not-fot-ptofit health system, a series of errors in 2006 resulted in a patient's death following a blood incompatibility event at Vidant Medical Center (VMC). Multiple visits by state regulators followed, and findings cited deficiencies in quality improvement and lack of oversight by the hospital's governing body. This serious safety event was pivotal in the quality journey of the health system, galvanizing the board and senior leaders to place intentional focus on patient safety, performance improvement, and patient experience. Specific methods, as described in this article, were used to drive the changes, including safety training, quality strategic planning, board leadership, transparency, patient-family partnerships, and leader and physician engagement.
