摘要

The future of quality and safety improvement relies on development of an appropriate supportive infrastructure, which Chassin and Loeb have described as a culture of high reliability. ' Specifically, a high reliability system will yield a learning organization, which, in turn, will design, improve, share knowledge, and develop problem-solving capabilities for the institution. Consequently, the stable future of health care remains dependent on a rational approach to the design of a "faster, better, cheaper" environment.' Central to this paradigm is the use of accurate information to direct our efforts toward both performance and performance improvement.
