CSB REPORT ON CARBIDE

The CSB has published its report on the Carbide Industries explosion that killed two workers and injured two others in Louisville, KY, on March 21 2011. The large explosion at Carbide Industries resulted from a failure by the company to investigate similar but smaller explosive incidents over many years while deferring crucial maintenance of the large electric arc furnace that blew up, according to the US Chemical Safety Board.
