An employer having workers who are entitled to the minimum wage or to both the minimum and overtime wages prescribed by the FLSA must maintain and preserve records that show the following for each such worker (29 CFR 516.2):rn1. Name in full, including the employee's number or identifying symbol if such is used in place of a name on any time, work or payroll record. See ‖ 1213.rn2. Home address, including zip code. See ‖ 1213.rn3. Date of birth if the employee is under 19 years of age. See ‖ 1213.rn4. Sex and occupation in which the worker is employed. See ‖ 1213.rn5. Time and name of day on which the employee's workweek begins. See ‖ 1217.
