Introduction: Blastocyst transfers achieve higher implantation and live birth rates compared to cleavage stage embryos. To select the best blastocyst, a morphological grading system, described by Gardner and Schoolcraft (Gardner and Schoolcraft, 1999) has been widely used. It is based on grading three parameters; blastocoele expansion, size and compactness of the inner cell mass (ICM), and cohesiveness and number of trophectoderm cells (TC). Many investigators have shown that blastocysts with high scores for all three parameters achieve highest implantation rates. Unfortunately, either due to multiple transfers or grouping of blastocysts with variations of alphanumeric scores, the independent affect of each parameter on pregnancy outcome is lost and/or not reported. We have been performing single blastocyst transfer for a number of years and had the unique opportunity to use our patient database to analyse the outcomes of each blastocyst. The objective of this study was to compare each grading parameter as independent predictors of pregnancy/live birth outcome after single embryo transfer.
