For the first time in the 30-year history of AIDS we have the resources, the knowledge and the political will to mount a truly transformative response. From the early days of the pandemic it was clear that social inequality and margin-alisation were fuelling its growth. Nevertheless, a crisis mentality pervaded the response, funnelling resources towards the search for vaccines and treatment and the development of behavioural modification programmes to provide individuals with the information and tools to protect themselves from infection. While these investments helped save lives and protect public health, they have been ultimately insufficient to contain the growth of the pandemic in our complex and increasingly interconnected world.
