Recently, Huang and his colleagues published an interesting study entitled “Clinical outcomes and prognostic factors for prolonged mechanical ventilation in patients with acute stroke and brain trauma” in Journal of the Formosan Medical Association. 1 The study of Huang et al. displays that reduced RCC GCS scores (first day of RCC GCS score compared with GCS score upon initial admission) is an independent prognostic factor for weaning failure in patients with acute stroke and brain trauma requiring PMV. The grouping of patients in this article is not ideal. The weaning failure PMV patients included RCC mortality patients and ventilator dependent patients so that the two groups of patients have different clinical characteristics.
