Anterior cruciate ligament (ACL) injuries are a common sports injury in the young active patients, and concomitant varus defor- mity and excessive posterior tibial slope have been regarded as a predisposing risk factor for graft failure and progression of knee osteoarthritis after surgery. 1,2 Currently, isolated ACL reconstruc- tion fails to correct increased posterior tibial slope and varus mala- lignment, which may result in ACL graft early failure, because steep posterior tibial slope can yield excessive loading forces on recon- structed ACL graft. 3 In addition, varus malalignment adds an adduc- tion moment while patients walking, which can translate the varus thrust on the knee joint during the heel strike. 4 Given that these osseous deformity factors are the common contributing risk factors for ACL graft failure and the development of medial compartment knee osteoarthritis, we presented our surgical technique in terms of the clinical application of single-stage ACL reconstruction com- bined with valgus high tibial osteotomy, and make an introduction of its steps of surgical techniques.
