Commentary regarding 'Looped suction catheter in an i-gel?; something to worry about or much ado about nothing?'

Weread with interest thearticle'Looped suction catheter in an i-gel?;something to worry about or much ado about nothing?' published recently in theIndian JournalofAnaesthesia.[1] Theauthors havereported an interesting observation ofthelooping back ofthesuction catheter into theairway port ofthei-gel, which could lead topotentially disastrouscomplications, including gastricaspiration, if overlooked. Werecently experienced asimilarclinicalsituation ofa patient undergoing laparoscopiccholecystectomywith i-gelin-situ, wherein theinserted suction catheter through the gastric port ofan i-gel? (size 4) was found misdirected and emerging retrogradethrough theairway channel. However, unliketheauthors, in ourcase, the misdirected cathetercould not beseen through thetransparent body ofi-gel during thesurgery and was found only at theend ofsurgery [Figure 1]. Thereareseveraladditional points ofclinicalsignificance whichwe want to discuss through this letter.
