Miltefosine-induced acute interstitial pneumonitis in a patient with renal dysfunction

Miltefosine is the only oral treatment for visceral leishmaniasis. Gastrointestinal discomfort and elevation of serum creatinine and transaminases are its known side-effects.1 Hemoptysis and dyspnea have been described in phase IV trials,2,3 but an association between miltefosine and the development of lung toxicity has not been demonstrated. To the best of our knowledge, this is the first description of a causal relationship between miltefosine and pneumonitis.
