I have read with great interest the study by Lei et al., aboutthe impact of concomitant dyspepsia and irritable bowelsyndrome (IBS) on symptom burden in patients withgastroesophageal reflux disease (GERD). The authorsdescribed carefully the esophageal findings obtained duringupper gastrointestinal endoscopy. 1 However, they did notreport the gastric findings obtained in the same context.This raises a criticism due to the fact that the termdyspepsia includes several organic diseases (ranging fromgastritis to cancer) as well as the condition called func-tional dyspepsia (FD). The latter includes all patients inwhom no explanation for the symptoms can be identified bytraditional diagnostic procedures. 2 These features havedifferent pathogenic mechanism with different therapeuticapproaches. Thus, to cure a specific organic gastric diseasecould be different that the treatment of a FD with over-lapping IBS and GERD. 3 In conclusion, this interesting workwould be enriched with detailed information on gastricstatus.
