Introduction: Establishing patient safety culture is the first step in patient safety improvement. Therefore we need an assessment of patient safety culture in hospitals. Patient safety culture is part of the organizational culture, so that the study of organizational culture needed to be guidance in developing patient safety. This study aims to determine the patient safety culture among clinical staff in one hospital in Jakarta and to identify the organizational culture profiles. Materials and Methods: This study used cross-sectional design, with descriptive and qualitative analysis, conducted in 2014 in one hospital in Jakarta, Indonesia. Respondents consisting of nurses, midwives, physicians, pediatricians, Obgyn specialists, laboratory staff and pharmacy staff (n=152). The questionnaires used are Hospital survey on Patient Safety Culture by Agency for Healthcare Research and Quality (AHRQ) and Organizational Culture Assessment Instrument (OCAI) Results: Study found that teamwork within the units is the strongest dimension of patient safety culture (91.7%) while staffing and non-punitive response are the weakest dimensions (22.7%). Clan culture is the type of the current organizational culture which is dominant and considered as a strong culture. This type is also the preferred culture that serves as a guide to make changes in the organization, especially in the development of patient safety. Conclusion: Hospital staff feel positively toward patient safety culture within their organization. Plan of action was made by Consensus Decision Making Group and achieved the agreement to articulating patient safety in the vision and mission of the organization and strengthening safety culture through patient safety training for all staff.
