Rare bile duct anomaly: B3 duct draining to gallbladder

A 10-year-old girl presented with recurrent right upper abdominal pain and dyspepsia. Magnetic resonance cholangiopancreatography revealed a dilated common channel of intrahepatic bile duct of segment 3 (B3) and segment 4 (B4) drained into the gallbladder directly. The patient underwent laparoscopic cholecystectomy and Roux-en Y hepaticojejunostomy (B3-jejunostomy). Among the anatomical variability of the biliary system, the cholecystohepatic ducts are controversial in existence and incidence. We report a very rare variant of a cholecystohepatic duct in which the B3 duct drained into gallbladder directly and to the best of our knowledge this is the first report. Highlights ? The cholecystohepatic ducts, which are also called the pericholecystic ducts, are very rare bile duct anomaly. ? We described a rare variant of a cholecystohepatic duct in which the B3 duct drained into gallbladder directly. ? To the best of our knowledge the present anomaly has not been previously reported in the literature.
