Objective: To investigate the relationship between changes of coagulation function and plasmacTnI, HCY, PCT levels and cerebral trauma severity and prognosis in patients with acutecraniocerebral injury. Method: A total of 80 patients with acute craniocerebral injury treated inour hospital from May 2014 to May 2017 were selected as the observation group and dividedinto 3 groups according to the admission GCS score: mild (27 cases), moderate (28 cases) andsevere group (25 cases). 40 healthy volunteers were selected as the control group. The index ofblood coagulation (APTT, PT, Fib, TT, DD) and the levels of cTnI, HCY, PCT in observationgroup after injury and in control group were detected and analysed comparatively. After 1months of treatment, 80 patients were divided into the good prognosis group (54 cases) andpoor prognosis group (26 cases) according to the GOS score and the levels of the parametersabove were detected. Results: (1) The PT, APTT, TT and DD levels of mild, moderate andsevere acute craniocerebral injury groups were higher than those of the control group, butthe level of Fib was lower than that of the control group, and there was also a significantdifference between any two groups respectively. (2) The cTnI, HCY and PCT levels of mild,moderate and severe acute craniocerebral injury groups were higher than those of the controlgroup, and there was also a significant difference between any two groups respectively. (3) Theindexes of PT, APTT and DD in the good prognosis group were lower than those in the poorprognosis group, while the Fib level was higher significantly. (4) The levels of cTnI, HCY andPCT in the good prognosis group were significantly lower than those in the poor prognosisgroup. Conclusion: The levels of coagulation function and plasma cTnI, HCY, PCT in acutecraniocerebral injury patients were correlated positively with the severity and prognosis ofpatients with craniocerebral injury.
机译:目的:探讨急性颅脑损伤患者凝血功能的变化与血浆cTnI,HCY,PCT水平的变化与脑创伤严重程度及预后的关系。方法:选择2014年5月至2017年5月在我院就诊的急性颅脑损伤患者80例作为观察组,根据入院GCS评分分为3组:轻度27例,中度28例,重度组(25例)。选择40名健康志愿者作为对照组。检测并比较了损伤后观察组和对照组的凝血指标(APTT,PT,Fib,TT,DD)和cTnI,HCY,PCT水平。治疗1个月后,根据GOS评分将80例患者分为良好预后组(54例)和不良预后组(26例),并检测上述各项指标的水平。结果:(1)轻,中,重度急性颅脑损伤组的PT,APTT,TT和DD水平均高于对照组,但Fib水平低于对照组,且有一定程度的升高。两组之间的差异显着。 (2)轻,中,重度急性颅脑损伤组的cTnI,HCY和PCT水平均高于对照组,两组之间也存在显着性差异。 (3)预后良好组的PT,APTT和DD指数低于预后不良组,而Fib水平明显升高。 (4)预后良好组的cTnI,HCY和PCT水平明显低于预后不良组。结论:急性颅脑损伤患者的凝血功能水平和血浆cTnI,HCY,PCT水平与颅脑损伤患者的严重程度和预后呈正相关。
