A43-year-old woman presented to theemergency department with acute rightlower quadrant abdominal pain,nausea, and bloating for the past day. Her medicalhistory included a Roux-en-Y gastric bypass pro-cedure 14 years prior. On abdominal examinationshe had focal guarding in her right lower quadrant.A computed tomographic scan of her abdomenand pelvis showed a multicystic heterogeneousright ovarian mass. Exploratory laparotomyrevealed a large right hemorrhagic ovarian cystwith ovarian torsion of the infundibular ligamentand vascular pedicle (image A, arrow). She under-went an oophorectomy.
