Congenital Adrenal Hyperplasia—More Dogma Bites the Dust

Now, with parallel advances in mass spectrometrometric methods for comprehensive ste- roid analysis (6), we have come full circle, and steroid chemistry has regained center stage. In this issue of the JCEM, Kamrath et al. (7) show how advanced technolo- gies applied to urinary steroid analysis— coupled with a broader understanding of the pathways of androgen syn- thesis—are revising how we conceptualize the origin of androgen excess in CAH. The methodologies employed in this work and the insight gained from the data provide new analytes (biomarkers) that should improve the diagnosis of CAH and the management of this complex disorder.
