The odontogenic keratocyst (OK) is a cyst of odontogenic origin,which fnal diagnosis is given from the clinical, radiological andhistopathological fndings. The most common treatment is surgicalenucleation. The objective of the present study was to report a caseof OK in the posterior mandible, approaching the clinicopathologicalfeatures and treatment for this lesion. A 69-year-old female patientpresented a tumor lesion of ulcerated surface in the retromolar regionof the right side. Radiographic examination of the affected regionrevealed the presence of radiolucent image with well-defned limits.After two months of the frst appointment and after having madeadjustments to her poorly adapted prosthesis, the patient returnedpresenting a tumor lesion, without the presence of ulcers, in the sameregion. Under the diagnostic hypotheses of OK and residual cyst,excisional biopsy was performed. The histopathologic fndings wereconsistent with OK. Eighteen months after the removal of the lesionthere were not identifed clinical and radiographic signs of recurrence.The OK has characteristic clinical and radiographic fndings, aswell as different therapeutic possibilities, being these approachesaggressive or conservative. In all cases, periodic follow-up isnecessary, taking into account the high recurrence rates of the disease.
