Reumatoidalne zapalenie stawów (RZS) jest przewlek??, uk?adow? zapaln? chorob? tkanki ??cznej o pod?o?u autoimmunologicznym, charakteryzuj?c? si? symetrycznym zapaleniem stawów, które prowadzi do rozleg?ych uszkodzeń i zniekszta?ceń aparatu ruchu. Coraz wi?ksza wiedza na temat mechanizmów prowadz?cych do rozwoju RZS przebiegaj?cego z aktywacj? prozapalnej odpowiedzi immunologicznej przyczyni?a si? do powstania nowej grupy leków biologicznych, do których nale?? antagoni?ci czynnika martwicy nowotworów α (tumor necrosis factor α – TNF-α). Zalicza si? do nich zarówno chimeryzowane (infliksymab), jak i ludzkie przeciwcia?a anty-TNF-α (adalimumab, golimumab), a tak?e fragment Fab przeciwcia?a ludzkiego, po??czonego z glikolem polietylenowym (certolizumab pegol) oraz bia?ka fuzyjne wi???ce si? kompetencyjnie z b?onowym receptorem TNF-α (etanercept). W wielu dotychczas przeprowadzonych badaniach klinicznych potwierdzono znaczne zmniejszenie aktywno?ci choroby, zahamowanie post?pu zmian stawowych stwierdzanych w badaniu radiologicznym oraz znaczn? popraw? wydolno?ci uk?adu ruchu i jako?ci ?ycia osób chorych na RZS leczonych inhibitorami TNF-α. W niniejszej pracy przedstawiono przegl?d aktualnie stosowanych antagonistów TNF-α w terapii RZS. Rheumatoid arthritis is a chronic, systemic autoimmune connective tissue disease characterized by the symmetrical joint inflammation, leading to erosion and destruction of the joint surface, which impairs their range of movement and leads to deformation of the joint. Recent years have brought improvement in the understanding of the pathogenesis of this disease, resulting in the production of a new groups of biological drugs, including, among others, anti-TNF-α antibodies. The latter include chimeric (infliximab) and fully human anti-tumor necrosis factor α monoclonal antibodies (adalimumab, golimumab), a pegylated Fab fragment (certolizumab pegol) and a recombinant fusion proteins that effectively bind the soluble TNF-α receptor (etanercept). The results of clinical trials revealed a significant reduction of disease activity and inhibition of radiological progression during treatment with TNF-α blockers.
