摘要

Assisted reproductive technology (ART) is defined as the general terminology of advanced treatmentfor infertility which includes in vitro fertilization and embryo transfer (IVF) and intracystoplasmicsperm injection. More than 4% of new born babies in 2013 came from the conceptions achieved byART in Japan. The history of ART started in 1978 from the birth of world first test-tube baby “LouiseBrown” achieved by IVF in the United Kingdom. Nobel Prize in Physiology and Medicine of 2010awarded late Professor Robert Edwards who developed the technology of human IVF which broughtmillions of babies from infertile couples all over the world. Moreover, his laureate has proven that IVFis effective and safe treatment for infertility. Japanese history of ART started in 1983 from Sendai wherelate Professor Masakuni Suzuki achieved the first successful IVF pregnancy and birth. Since then, number of ART cases in Japan has been steadily increasing and become the highest number of ART casesperformed in the world. Although ART was considered to be almighty for any infertility disorders atthe beginning, lately the pregnancy rate has been declining due to the increased number of olderpatient. Therefore, preimplantation genetic screening (PGS) of embryos are applied to older patient toselect chromosomally normal embryos for transfer in order to improve pregnancy rate and avoid miscarriage in the world. Otherwise, donor oocytes are substituted for their own oocyte to improve pregnancy rate in many other countries except for Japan. PGS is a strong tool to select normal embryos forchromosomal wise, but chromosomally normal embryos achieve only 60-70% of pregnancy rate, not100%. Status of nucleus and also cytoplasm is critical for implantation. The condition of cytoplasmlargely relies on the mitochondrial activity. It is well known that low-reactive level laser activates mitochondrial function. Although low reactive level laser treatment (LLLT) has not been well recognized inthe field of reproductive medicine, promising potential effect of LLLT should be utilized as a part ofintegrative medicine for infertility treatment. LLLT could play an integral role in integrative medicine inthe field of ART in the future.
