Sincethe Institute of Medicine’s report, “To Erris Human,” suggested that there were100,000 deaths annually from medical error,physicians, nurses, and hospital organizationshave worked to improve patientsafety.1,2,3 Medicine requires the integrationof motivated, intelligent practitionerswith highly complex, technical systems. Asimilar analogy exists within aviation. Duringthe early days of flight, accidents werecommon and blame was placed on failureof the machine.
