A Logic Model for Understanding and Reducing Preventable Hospitalizations

Identifying and understanding the root causes of pre- ventable hospitalization (PH) is important for improving health outcomes and reducing unnecessary healthcare costs. Thus far, however, the desire to address this issue has been impeded by a general lack of research on factors associated with PH. To begin to address this gap, we pro- pose an evidence-based logic model of individual, envi- ronmental, and systemic factors related to PH. We aim to use this logic model to design public health interventions to reduce PH in the State of Rhode Island and to stimu- late an industry-wide discussion of the problem and its possible solutions.
