Introduction. Surgery plays a central role in the managementof organ-confined prostate cancer (PCa). Fourtypes of prostatectomy are currently practiced: perineal,retropubic, laparoscopic, and robot-assisted. The qualificationcriteria for all types are similar. Radical perinealprostatectomy (RPP) was the first method introducedinto clinical practice, however, it has been neglectedin favor of other procedures. Its resurgence has beenfacilitated by a multitude of advantages. Unfortunately,nowadays most urologists are not familiar with theperineal approach though many centers have begun toimplement it.Materials and methods. This manuscript presents thetechnique of RPP used in the author’s institution. It alsoreviews a vast body of literature on the four techniquesof prostatectomy including their advantages and outcomes.The data was collected from the literature andmedical databases.Conclusion. RPP proves to be a very efficacious, costeffectivetreatment option for localized PCa. The outcomesof RPP, as defined by continence, potency, andcomplication rate, are equivalent to those accomplishedby other methods.
