'Fundamental' safety issues in rail

Network Rail has been told it must address "fundamental" safety issues for workers on rail lines that "have been highlighted time and again" by the Rail Accident Investigation Branch (RAIB). The watchdog said the rail operator must improve the quality of its onsite leadership, provide clear information on decision-making, and try to do away with the need for individual workers to warn of approaching trains.
