Cases: Failure to act on results of high vaginal swab leading to death: FA v Worcestershire Acute Hospitals NHS Trust

The Claimant's mother had had two previous miscarriages before she became pregnant with the Claimant in May 2003. On 9 April 2003 at approximately 29 weeks gestation the Claimant's mother had a high vaginal swab (HVS) taken as she thought her membranes might have ruptured the previous night. There was no clinical evidence of this and she went home. This HVS grew no pathogens on culture. nnShe was seen on 14 June 2003 with bilateral oedema of her legs below the knee and slightly swollen fingers. Blood tests were done and showed a haemoglobin of 10.6, white blood cell count (WBC) of 12.2 with 8.25 in platelets of 280. These results were normal for late pregnancy. The Claimant's mother was allowed home.
