Over the years, we've seen all manner of aircraft destroyed and their occupants lost in incidents stemming from fuel mismanagement - airliners, executive aircraft, military fighters and even crop dusters. The NTSB took special notice of this situation recently in its report on the Aug. 14, 2016, loss of a Piper Navajo Chieftain (PA-31-325, N447SA) that crashed into terrain near Tuscaloosa, Alabama, Regional Airport (KTCL) killing all six on board. The Safety Board determined the probable cause was "the total loss of power in both engines due to fuel starvation as a result of the pilot's fuel mismanagement, and his subsequent failure to follow the emergency checklist. Contributing to the pilot's failure to follow the emergency checklist was his lack of emergency procedures training in the accident airplane."
