RC, CBP and BPY had full access to all of the data in the study and take responsibility for the integrity of the data. RC takes responsibility for the accuracy of the data analysis. RC conducted data analyses and contributed significantly to data interpretation, developing the manuscript draft, and writing and revising the paper in response to co-author comments. CBP made substantial contributions to the conception and design of the study, oversaw data acquisition and reviewed the manuscript. RAM designed the patient-reported collection of assessments, contributed to the study conception and design, and participated in the data interpretation and reviewing of the manuscript. VPC contributed to the data interpretation and reviewing of the manuscript. NAP contributed to the study conception, data interpretation and revising of the manuscript.
