TOOLKIT TO ASSIST PRIMARY CARE PRACTITIONERS WITH THE RECOMMENDED 4-STEP KAER PROCESS

The GSA Workgroup created a compendium of tools and procedures to help PCPs implement the 4 steps in the KAER process: >Kick-start the conversation about brain health; >Assess for cognitive impairment>, Evaluate for dementia, and >Refer for community resources. The toolkit is includes a selection of tools and procedures so that PCPs in single-person offices, multi-physician practices, and health care systems will be able to choose the tools and procedures that fit best with their existing primary care practices. This presentation will discuss the structure and content of the toolkit, the topics for which appropriate tools were more and less difficult to find, and responses to the toolkit from PCPs and administrators of health plans and health care systems who participated in the GSA KAER Summit.
