The Therapeutic Drug Monitoring (TDM) program provides general and specific guidelines for drug therapeutics. Its flexibility allows applicability in many clinical settings of pediatric and adult medicine. TDM is written in Pascal and operates on an IBM PC. TDM has six subroutines. The “IV Infusion” option printouts individualize dosages and therapeutic footnotes for selected drip infusion medications. The “Kinetic Dosing” option lists basic pharmacokinetic parameters and individualized dosages for patients both with and without specific serum level data. The “Emergency Med List” gives patient specific dosages for crash cart medications. The “Hyperalimentation” routine individualizes TPN therapy for normal, uremic, and hepatic failure patients. The “Drug Overdose” option gives guidelines and information for using drug serum levels in overdose situations. A “Drug Interactions” option provides clinically significant drug interaction data. This TDM program will be a valuable practice and educational tool for clinicians interested in assuring rational, individualized drug therapeutics.
