腹腔镜左肝外叶切除术在基层医院的应用

摘要

目的：探讨腹腔镜左肝外侧叶切除术的手术方法，介绍手术中的操作技巧。方法回顾性分析2015年1月至2016年1月9例行腹腔镜左肝外侧叶切除术患者的临床资料，所有患者先使用血管夹结扎左肝动脉外叶分支，再用超声刀切开肝脏至左肝外叶肝蒂处后使用强生切割闭合器EC60白钉切除左肝外叶。结果9例患者均成功完成手术，出血量50~200ml，手术时间80~150min。结论使用腹腔镜下切割闭合器，腹腔镜左肝外侧叶的切除简单易行，值得在基层医院推广。%Objective To explore the surgical method of laparoscopic left lateral lobe resection.Describes the skills in surgery. Methods Analysis of the clinical data of 9 patients who underwent laparoscopic left lateral lobe resection from January 2015 to January 2016 was performed. All patients had firstly vascular clamp on left hepatic artery ligation of the branches,then harmonic scalpler was applied to resect the liver paranchyma till the triat of left lobe was exposed, before it was managned withEndo-GIA Stapler EC60 with white load to complete the resection of left lateral lobe. Results 9 cases of patients were successfully performed with the operation,the bleeding was 50 ~ 200 ml,the operation time was 80~150min. Conclusions Using laparoscopic Endo-GIA Stapler,laparoscopic left lateral lobe resection is simple,is worth popularizing in basic-level hospitals.
