Objective To compare the therapeutic effect of laparoscopic operation and open surgery in treatment of varicocele. Methods 55 patients with varicocele were selected and divided into two different treatment groups. 30 cases were in laparoscopy group and 25 cases were in open surgery group. The therapeutic effect of all patients was summarized and compared. Results Operative time and hospital stay of the laparoscopy group were significantly improved compared with the open surgery group(P<0.05). The improvement of symptoms and sperm quality of two groups had no significant different(P>0.05). All operation results were fine,with no serious complications. Conclusions Laparoscopic operation has the same curative effect as traditional open surgery in treatment of varicocele with less surgical trauma and shorter hospital stay,especially suited for simultaneous treatment of bilateral lesions,which has a good applied future in the clinical practice.%目的：比较精索静脉曲张使用腹腔镜和开放手术两种不同治疗方法的临床效果。方法精索静脉曲张患者54例，使用两种不同手术方式治疗，腹腔镜组30例，开放手术组24例，比较两种不同术式治疗的临床效果。结果腹腔镜手术组患者在手术时间，住院时间与开放手术组比较有明显优势（P<0.05）；腹腔镜手术组和开放手术组两者在术后症状改善、并发症发生及精子质量改善等方面比较差异无统计学意义（P>0.05）。结论腹腔镜和开放手术治疗精索静脉曲张均安全有效。腹腔镜手术创伤小，可缩短手术时间及住院时间，尤其适合于双侧精索静脉曲张的治疗，值得临床进一步应用。
