病毒性脑炎患儿不同病程血浆维生素A水平的变化特点

         

摘要

目的 观察病毒性脑炎(VE)患儿不同病程血浆维生素A(VA)水平的变化特点.方法 选取VE患儿63例,另将多发性抽动症(TS)患儿48例、癫痫(EP)患儿45例及健康儿童40例为对照组,测定VE急性期、恢复期的血浆VA水平,同时与3组对照组进行比较,分析VE急性期维生素A缺乏(VAD)发生率及VA水平与临床特征及相关实验室检查数据的关系.结果 VE急性期的VAD发生率显著高于恢复期(P<0.001),血浆VA水平显著低于恢复期(P<0.001);VE患儿急性期的VA水平均显著低于TS对照组、EP对照组及健康对照组(P<0.001),恢复期VA水平与TS对照组、EP对照组及健康对照组比较差异无统计学意义(P>0.05);VE急性期VAD发生率及VA水平与临床特征及相关实验室检查数据均无明显相关性(P>0.05).结论 VE患儿在急性感染期其血浆VA水平显著降低,恢复期可基本达到正常儿童水平,该变化特点不受患儿临床特征及相关实验室检查等因素的影响,VAD可能是VE发病的独立影响因素之一.%Objective To investigate the changes of plasma vitamin A (VA) levels in children with viral encephalitis (VE). Methods 63 children with VE were selected as the subjects. Forty - eight children with multiple tic disorder (TS),45 children with epilepsy (EP) and 40 healthy children were enrolled in this study. The plasma VA level in the acute phase and the recovery period of VE was measured and compared with the three control groups. The relationship between the prevalence of vitamin A deficiency (VAD) and the level of VA and the clinical characteristics and related laboratory data were analyzed. Results The prevalence of VAD in VE acute stage was significantly higher than that in convalescence (P<0.001),plasma VA level was significantly lower than recovery period (P<0.001);VE children with acute stage of VA were significantly lower than TS control group,EP control group and healthy control group (P<0.001). There was no significant difference between the control group and the control group (P>0.05). The VAD prevalence rate in VE acute stage was significantly higher than that in TS control group,EP control group and healthy control group. VA levels were not significantly correlated with clinical features and related laboratory data (P>0.05). Conclusion The plasma VA level of children with VE was decreased significantly in the stage of acute infection,and the recovery period can reach the normal children level. The change characteristics are not affected by the clinical features and related laboratory tests. VAD can be one of the independent factors In VE.
