目的 比较锁定钢板固定与交锁髓内钉内固定治疗AO-C型股骨干粉碎性骨折的疗效.方法 随机选取2012年1月至2016年1月AO-C型股骨干粉碎性骨折患者96例,按照手术固定方式不同,分为钢板组(n=48)与髓内钉组(n=48),髓内钉组患者采用交锁髓内钉内固定,钢板组患者采用锁定钢板固定,比较两组患者的临床疗效及并发症发生情况.结果 髓内钉组患者的手术时间、住院时间、骨折愈合时间均显著短于对照组(P＜0.05),术中出血量显著少于对照组(P＜0.05);且髓内钉组患者的优良率显著高于对照组(P＜0.05),并发症发生率显著低于对照组(P＜0.05).结论 采用交锁髓内钉内固定治疗AO-C型股骨干粉碎性骨折,能够显著缩短患者的手术时间、住院时间及骨折愈合时间,减少患者术后出血量,且术后并发症发生率较低,临床治疗效果良好,值得临床推广使用.%Objective To study the clinical effect of locking plate fixation and interlocking intramedullary nail fixation in the treatment of comminuted fracture of femoral shaft.Methods 96 cases of comminuted femoral shaft fractures from January 2013 to January 2016 in our hospital were treated according to different fixation methods and divided into the observation group (n=48) and control group (n=48), the observation group was treated by intramedullary nail fixation, the control group was treated with locking plate fixation, the clinical efficacy and complications were compared between two groups.Results The observation group of patients with operation time, hospitalization time, fracture healing time were significantly shorter than that of the control group (P＜0.05), the amount of bleeding was significantly less than that of the control group (P＜0.05);and the excellent rate of observation group was significantly higher than that of control group (1P＜0.05), the incidence of complications was significantly lower than that of the control group (P＜0.05).Conclusion The use of interlocking intramedullary nail in the treatment of comminuted fracture of femoral shaft, can significantly shorten the operation time, hospitalization time, fracture healing time, reduce the amount of bleeding after surgery, and the incidence of postoperative complications is low.It has good curative effect and worthy of clinical application and promotion.
