碱性成纤维细胞生长因子联合低氧诱导因子1对提高随意型皮瓣缺氧耐受的实验研究

         

摘要

目的 探讨纳米微囊包裹碱性成纤维细胞生长因子(bFGF)联合低氧诱导因子1(HIF-1)对提高大鼠随意型皮瓣缺氧耐受的影响和作用机制.方法 根据改良大鼠McFarlane皮瓣制作方法,在28只健康SD大鼠的背部设计随意型皮瓣.在距皮瓣蒂部近、中、远三处局部分别注射药物.根据注射药物不同分为4组,纳米微囊包裹bFGF联合HIF-1组(实验组)、纳米微囊包裹bFGF组(对照组1)、纳米微囊包裹HIF-1组(对照组2)和纳米微囊组(空白对照组).术后第7天处死大鼠,分别测量皮瓣的成活面积、HE染色及目的蛋白免疫组化表达情况及皮瓣新生微血管密度(MVD)检测等.结果 术后第7天,实验组皮瓣存活率(92±5)%,MVD(44±7)个,均高于其他三组,差异有统计学意义(P<0.05).实验组中皮瓣组织水肿和炎症细胞浸润情况明显比其他各对照组减轻.实验组,对照组1和对照组2中能观察到目的蛋白的表达,而空白对照组中无表达.结论 以纳米微囊为载体包裹bFGF和HIF-1,能显著提高大鼠随意型皮瓣的缺氧耐受,增加皮瓣成活面积,促进皮瓣存活.%Objective To investigate the effect of nano-microcapsules encapsulated basic fibroblast growth factor (bFGF) combined with hypoxia-inducible factor-1 (HIF-1) on increasing the survival area of rat' s ischemia random flaps and its mechanism.Methods McFarlane flap model was established on the rat dorsum.Twenty-eight Sprague Dawley rats were used for this experiment.Local and homogeneous injections were performed in the proximal, middle and distal end of the flaps.The flaps were divided into 4 groups equally according to the different drugs:Nanomicrocapsules encapsulated bFGF combined with HIF-1 (experimental group), nano-microcapsules encapsulated bFGF (control group 1), nano-microcapsules encapsulated HIF-1 (control group 2) and nano-microcapsules (blank control group).The measurement of the survival area of the flaps, HE staining, the expression of interest protein and the detection of neoformative microvessel density (MVD) were performed at 7 days.Results On postoperative day 7, the percentage of the flap survival area of the experimental group was (92 ± 5) %, and the MVD was (44 ± 7).These results were significantly higher than that of control groups (P<0.05).Tissue edema and inflammatory cell infiltration observed in the experimental group were obviously reduced than those of the control groups.The expression of interest protein was observed in experimental group,control group 1 and control group 2.Conclusion The nano-microcapsules encapsulated bFGF combined with HIF-1 can obviously increase the survival area of ischemia random flaps.
