慢性肾脏病湿热型体质易感人群ICAM1基因和IL-10基因启动子多态性研究

         

摘要

目的:探讨岭南地区湿热型体质慢性肾脏病 (CKD) 1期患者ICAM1基因和白细胞介素10 (IL-10) 基因启动子的多态性.方法:选择106例CKD1期患者, 随机分为对CKD1期湿热质组 (50例) 、CKD1期平和质组 (56例) ;选择102例健康体检者, 按体质分为健康湿热质组 (41例) 、健康平和质组 (61例) .利用荧光定量PCR方法和ELISA检测方法进行IL-10基因启动子-1082、-819、-592位点多态性研究及ICAM1基因G241和K469位点的多态性分析, 并且检测ICAM1、IL-10血清浓度及对比分析.结果:4组IL-10基因启动子、ICAM1基因各位点基因型分布及基因频率比较, 差异无统计学意义 (P>0.05), 4组IL-10血清浓度比较差异无统计学意义 (P>0.05), 4组ICAM1血清浓度比较差异有统计学意义 (P<0.05), 且含有G241位点GG基因型血清ICAM1浓度显著比含有GR基因型低, 差异有统计学意义 (P<0.05), 含有K469位点不同基因型血清ICAM1浓度比较差异无统计学意义 (P>0.05) .结论:湿热体质人群对CKD具有易感性, ICAM-1 G241位点GG基因型可能是CKD湿热体质预测基因, 且通过G241位点GG基因型调控ICAM1, 使得ICAM1的表达下调, 血清ICAM1浓度降低, 减少炎症反应.%Objective:To discuss the promoter polymorphism of ICAM1 gene and IL-10 gene of damp-heat constitution patients suffering chronic kidney disease (CKD) at stage one in Lingnan district. Methods:All 106 CKD1 patients were chosen and randomized into the group of damp-heat constitution CKD1 (50 cases) and the group of peaceful constitution CKD1 (56 cases) ; another 102 people who accepted physical examinations were selected and randomized into health group of damp-heat constitution (41 cases) and health group of peaceful constitution (61 cases) . Polymorphism of ICAM1 gene G241 and K469 loci were analyzed and polymorphism of IL-10 gene promoters-1082, -819 and-592 loci were studied by fluorescent quantitation PCR methods and ELISA method, the concentrations of ICAM1 and IL-10 were detected, compared and analyzed. Results:The difference had no statistical meaning in the comparisons of genotype distribution and gene frequencies of IL-10 gene promoter and ICAM1 gene (P>0.05), the difference had no statistical meaning in the comparisons of the concentrations of IL-10 among four groups (P>0.05), the difference was statistically significant in the comparisons of the concentrations of ICAM1 among four groups (P<0.05), and the concentrations of ICAM1 in GG genotype serum containing G241 loci were lower than these in GR genotype serum, and the difference had statistical meaning (P<0.05), the difference had no statistical meaning in the comparisons of the concentrations of ICAM1 in different genotypes serums containing K469 loci (P>0.05) . Conclusion:The people in damp-heat constitution have the susceptibility to CKD, ICAM1-1 G241 loci GG genotype might be the gene predicting damp-heat constitution CKD, and it could regulate ICAM1 by GG genotype G241 loci, reduce the expressions of ICAM1, decrease the concentrations of ICAM1 and relieve inflammatory reaction.
