四君子汤经不同途径给药对窒息后喂养不耐受新生儿血浆胃动素的影响

         

摘要

目的:观察不同给药途径下四君子汤对窒息后喂养不耐受新生儿血浆胃动素(MTL)的影响.方法:将78例窒息后喂养不耐受新生儿随机分为四君子汤经胃管微量给药组、四君子汤微量灌肠给药组及常规静脉高营养形式喂养的对照组各26例,分别施以四君子汤微量喂养、四君子汤微量灌肠及常规静脉高营养形式喂养,检测治疗前及治疗第1、3、4天患儿血浆MTL的变化情况,并与正常新生儿MTL比较.结果:新生儿窒息后多在开奶后的24~72小时内出现喂养不耐受的症状,其血浆MTL水平显著低于正常新生儿(P<0.01),经加用四君子汤治疗后,其血浆MTL水平呈逐渐回升的趋势并渐趋于正常,其恢复情况明显优于对照组(P<0.01);四君子汤经胃管微量给药组与经肛管灌肠给药组患儿在治疗后的不同时间段内血浆MTL水平的动态变化没有统计学意义(P>0.05).结论:在常规胃肠外营养的基础上加用四君子汤治疗喂养不耐受能够对胃肠道激素分泌水平恢复正常起到有效的促进作用,从而加速疾病的康复.同时,经灌肠给药的方式相比其他给药途径更为简便易行,具有良好的可操作性,有利于临床推广.%Objective:To observe the different administration ways of SiJunZiTang on feeding intolerance after asphyxia of newborn in plasma motilin (MTL). Method:78 cases of asphyxia of newborns were randomly divided into groups SiJunZiTang feedings,enema therapy group and control group, 26 cases per group,they were respectively treated with SiJunZiTang,SiJunZiTang micro enema and conventional intravenous feeding in the form of high nutrition, before treatment and 1,3,4 days of treatment,to test the changes of plasma MTL and compared with normal newborns. Result:The feeding intolerance symptoms are appearing on the 24 to 72 hours after eating milk after asphyxia, the plasma MTL was significantly lower than the normal newborns(P<0.01),by addition of SiJunZiTang,its MTL plasma level was gradually tends to increase and gradually become normal,the recovery was significantly better than the control group (P<0.01);But there was no statistical difference in tube micro injection group and anal enema administration group in MTL plasma levels of different periods(P>0.05).Conclusion:It plays an effective role in promoting gastrointestinal hor mone levels through the conventional parenteral nuntrtion based on the use of SiJunZiTang to treat feeding intolerance,thus speeding up the rehabilitation of the disease.Meanwhile,the way is simpler and easy to use in clinical practice.
