中医辨证分型与乳腺癌预后的相关性研究

To explore the relationship between recurrence and metastasis of breast cancer and syn-drome differentiation and pattern identification. Methods: Statistical analysis was made on 120 cases with breast can-cer that met case standard. All cases were classified as four patterns:QiandYindeficiency pattern, water and damp-ness retention pattern,Qistagnation and blood stasis pattern and disharmony of Chong and Ren pattern. Results: A-mong 120 cases, cases withQistagnation and blood stasis pattern showed highest risk of recurrence and metastasis, with rate of recurrence and metastasis of 34.4% in one year and 84,4% in three years; there was significant difference when compared to cases with other patterns (P<0.001). There was no obvious relationship between tumor size and pathological types and syndrome differentiation and pattern identification (P>0.05). Her-2 receptor status was relat-ed to syndrome differentiation. When we compared four different patterns, there was statistical significance between them (P<0.05); there was significant difference between cases with disharmony pattern and cases with other pat-terns (P<0.05). Conclusion: Postoperative disease-free survival time of cases with breast cancer and Her-2 receptor status are related to syndrome differentiation and patterns of traditional Chinese medicine, while tumor size and its pathological type are not obviously related to syndrome differentiation and pattern identification. And cases withQi stagnation and blood stasis pattern have higher risk of recurrence and metastasis.%目的：探讨乳腺癌患者术后复发转移与中医辨证分型之间的关系。方法：采用回顾性分析的方法，共采集符合病例纳入标准的乳腺癌女性患者120例,分为气阴两虚、水湿内停、气滞血瘀及冲任失调4型，进行归纳统计分析。结果：120例女性患者中，气滞血瘀型的患者复发转移率最高，1年复发转移率为34.4%，3年复发率为84.4%；与其他3型相比，差异有统计学意义（P＜0.05）。肿瘤大小、不同的病理类型与中医辨证分型均无显著相关性(P＞0.05)。Her-2受体状态与中医辨证分型有一定的相关性，4种分型之间相比，差异具有统计学意义（P＜0.05）；其中冲任失调型与其他3型相比，差异有统计学意义（P＜0.05），而其余证型之间两两比较，差异均无统计学意义（P＜0.05）。结论：乳腺癌患者术后无病生存时间以及Her-2受体状态可能与中医辨证分型具有相关性，肿瘤大小、病理类型与辨证分型无明显相关性。其中气滞血瘀型的2患者发生复发的可能性较大。
